NOTE:

Do NOT begin filling out the forms until you have saved them to your computer. To do this, right click on the form (Mac users Control+click) and use the “save link as” option or “save target as” option to download document to desktop (or any other computer file or disk). Then open document by double-clicking (left button) on it.

ANOTHER OPTION is to open the form in Adobe Acrobat and click the icon in Adobe to “save a copy of the file” (looks like a floppy disk and is next to the PRINT option).
